The Contractor has not conducted the soil Field Density Test for road embankment, between CH0+700 - CH2+000 & CH7+300 - CH7+500 respectively, for ongoing road embankment earth filling works (refer attachment 1.0). Soil Field Density Test shall be conducted for each compacted layer before proceeding the next stages of works, as referred to the Standard Specification For Road Works (JKR/SPJ/2013-S2); Section 2.2; Sub-Section 2.2.4.4;"Compaction".

The contractor did not meet the required design strength (40 N/mm²) and target mean strength (47.38 N/mm²) for the G40N concrete grade in the 28-day cube compressive test. The average result was only 38.3 N/mm², with individual values of 34.7, 52.9, and 27.4 N/mm² for the 3 samples. (refer attachment 1.0 & 2.0)

Thus, the contractor must propose and submit a rectification works, endorsed by a Professional Engineer with Practicing Certificates, to the Superintending Officer for review and consideration before proceeding with the next stage of work.

The contractor is reminded to ensure that all concrete materials delivered to the site comply with the construction drawing, contract specifications (JKR/SPJ/2009-S9), and approved design mix.

The contractor failed to provide the protuded 75mm length (from the cut-off level to the top surface of the lean concrete) for the exposed RC pile head. Additionally, the constructed lean concrete was found to be damaged. (Refer attachment 1.0)

Thus, the contractor must propose and submit a rectification works, endorsed by a Professional Engineer with Practicing Certificates, to the Superintending Officer for review and consideration before proceeding with the next stage of work.

The contractor is reminded to ensure that all construction materials delivered to the site comply with the construction drawing and contract specifications (JKR/SPJ/2008-S9) accordingly.

Referring to your letter (GASB/JKR/GDSM/L138) & our letter (PTSB/JKRSMRH/SEMALATONG/2025/023), it was found out that the test results of the gradation limit is not comply with the specifications. Furthermore, these materials were delivered and placed on the existing plantation roads as part of the embankment fill for the proposed road alignment before the official test results were received as proposed by the contractor. (Refer to Site Memo No. 22)

This issue must be addressed as a priority. Please carry out all necessary corrective actions and ensure they are completed within 14 days from the date of this notice.

Kindly be reminded that the Method Statement and Material submission must be approved prior to proceeding with the next stage of the work.

It has come to our attention that earth filling works for the road embankment between CH19+425 and CH19+125 have been carried out without prior approval from Superintending Officer. (refer attachment 1.0)

Kindly take note of the following comments and observations:

1. Submission of locality plan and permit for the borrow pit is still pending.

2. Suitability test results for the borrow pit material have not yet been submitted.

3. Approval Chit for the backfilling activity is pending.

4. Drainage layer provided prior to backfilling is of insufficient width.

Please be reminded that it is the contractor's sole responsibility to ensure that all materials used and works executed are in full compliance with the approved contract specifications and construction drawings. Failure to comply with the above requirements shall be deemed as carrying out unauthorized works. The contractor will bear full responsibility for the consequences of such actions, including but not limited to any liabilities, defects, or claims that may arise.

Implementation of appropriate rectification measures shall be carried out without delay.
